1. 父母/监护人, go to the Final形式 website
2. Type your name, date of birth, and email and then click 注册
3. Check your email for a Final形式 email, and click confirm your account 在邮件文本中.
   • You will receive an email within 2 minutes prompting you to confirm and complete your registration. Remember to check your spam folder. If you still do not see the Final形式 email, please email support@finalforms.com.
4. Create your new Final形式 password and click 确认帐户.
5. 点击 注册学生 for your first child. Add your student and their email.

If you require any support during the registration process, scroll to the bottom of the Final形式 page and click “Use Support”.

Physicals are valid for 24 consecutive months

• 的 历史 document (Page 1) should be completed and remain with your provider
• 的 考试 document (Page 2) should be completed and remain with your provider
• 的 医疗资格 document (Page 3) should be the only item submitted to your school

Student-athletes must have health insurance to play a sport. If not covered by a parent/guardian’s insurance plan, insurance can be purchased through SPS.
